We have provided a list of financial aid forms that are most commonly accessed. All forms must be sent from the student’s Indiana Tech email address account if an electronic signature is included. If the forms are handwritten and/or sent from a non-Indiana Tech email address, the forms must have a wet handwritten signature. - This form must be printed out and signed in front of a notary or Indiana Tech staff member. The form must then be mailed to the Financial Aid Office in Fort Wayne, Indiana with a legible copy of the student’s government issued photo ID must accompany the form in the mail. The form and ID must be sent together or the document will be marked as incomplete.
